MISSION STATEMENT

We have a VISION of empowerment, choice, and community:

 

      People experiencing disabilities will live in a community, work at a job, and recreate, worship and volunteer in a variety of settings using the needed services of providers of choice.

 

     Therefore, Buchanan County is dedicated to providing funding for appropriate and cost-effective, community-based mental health and developmental disabilities services for all citizens. 

 

Central Point of Coordination (CPC)

 

WHAT IS THE PURPOSE OF THE CENTRAL POINT OF COORDINATION (CPC) OFFICE?

 

The CPC Office is the single point of entry for individuals living in Buchanan and wanting to receive mental health or disability services. The office will assist you with the following:
· To find the services you need
· To find a way to pay for these services


WHO IS ELIGIBLE FOR FUNDING FROM THE CPC OFFICE?

 

ALL residents are eligible for information and referral services through the CPC office.

To be eligible for additional funding through the CPC office individuals must meet ALL of the following criteria:
· Have a diagnosis of mental illness, mental retardation, developmental disability, or brain injury
· Meet specific income and asset guidelines
· Have legal settlement in Buchanan County
· Have a professional assessment determining that the services are needed


WHAT SERVICES ARE FUNDED BY THE CPC OFFICE:

 

Services funded include, but are not limited to the following:
· Outpatient mental health services (therapist, psychiatrist, community support programs, medication management, emergency services)
· Residential services (state institutions, residential care facilities, group homes, individual homes/apartments)
· Vocational Services (sheltered workshop, work activity, job coaches/supported employment)
· Respite Care
· Commitment Cost (attorney, sheriff's fee, hospitalization)
· Case Management/Social Work
· Adult Day Care


HOW DO I APPLY FOR FUNDING?

Applications must be returned to the CPC Office at 210 5th Ave NE, Independence, IA 50644.


HOW DO I KNOW IF I AM ELIGIBLE FOR FUNDING?

 

Once you have completed and returned the form, your application will be processed. Within 10 working days, a letter will be sent to you either approving funding, denying funding, or requesting additional information.

 

WHAT IF THIS IS AN EMERGENCY?

 

Buchanan County contracts with Backbone Area Counseling Center for 24-hour emergency services. This service can be accessed by calling their toll-free number at 1-877-847-0936.

 

Case Managment

 

TITLE XIX TARGETED CASE MANAGEMENT

 

Title XIX Targeted Case Management is when a Case Manager is actively involved in the process of assessing the abilities, obstacles, desires, needs, and wants of an individual with a disability.
The Case Manager takes the information they gather and uses it to make appropriate referrals and coordinates any needed services.
Once the services are in place the Case Manager then monitors the providers to ensure the individual's needs are being met in an appropriate manner.
Additionally the Case Manager serves as an advocate for the individual to ensure they are not taken advantage of and that their voice is being heard. Our office currently does Targeted Case Management for the following programs: MR Waiver, BI Waiver, Habilitation Services, and those with Developmental Disabilities.


WHO QUALIFIES?


· children who have applied and are eligible for the Mental Retardation Waiver through DHS
· adults 18 and over who have Title XIX and have a diagnosis of Mental Retardation, Chronic Mental Illness, Brain Injury, or Developmental Disability, and demonstrate a need for services
